Please complete all information on the ATM/Debit Card Application, print off, sign, and submit to us. You must have an open account, in good standing, with Galaxy to request a card. Click here for a copy of the Electronic Fund Transfers Agreement and Disclosure.
Failure to answer all information will delay the process. Completed applications may be dropped off, mailed, faxed, or emailed to us. Please be sure to provide a telephone number where you can be reached for questions.
